Facility Evaluation Report
Licensing Program Analyst (LPA) Angela Barutyan conducted a Case Management - Incident visit at 09:55AM regarding a self-reported incident which took place on 04/18/2025. LPA met with Administrator Crystal Cortes and explained the reason for the visit.
On 04/19/2025, the Department received an incident report and SOC341 from the facility stating that on 04/18/2025 around 02:30PM, Resident #1 (R1) alleged sexual assault by Staff #1 (S1). At the time, R1 was refusing to go to their scheduled appointment and when asked why R1 did not want to go, R1 stated that they “did not want to go with [S1]". R1’s responsible party and North Los Angeles County Regional Center (NLACRC) were immediately notified; R1 refused a medical examination but agreed to go to their scheduled appointment. After R1 returned to the facility, law enforcement was notified and arrived at 09:50PM to open an investigation on the incident. R1 agreed to a medical examination and was taken by law enforcement. R1 is being frequently monitored every thirty (30) minutes and S1 has been suspended pending investigation.
During LPA's visit today, LPA toured the facility, interviewed Administrator at 09:58AM, and obtained copies of pertinent documents. No immediate health and safety hazards were identified during facility tour.
Facility management was informed that this incident was referred to Community Care Licensing Division's Investigations Branch (IB). LPA and/or IB Investigator will return at a later date regarding this incident.
No deficiencies cited during today's visit. Exit interview conducted. A copy of the report was provided.
