Facility Evaluation Report
Licensing Program Analyst (LPA) Quoc Huynh arrived at the facility unannounced to conduct a required annual visit at 1:07PM. The LPA met with Administrator Zenaida Velasco who stated they did not have any residents. Entrance interview conducted.
The LPA toured the physical plant areas inside and outside with the Administrator at 1:12PM.
KITCHEN: Appliances in the kitchen appeared functional. The supply of perishable and nonperishable food is adequate. LPA observed a locked drawer and cabinet for knives and medications.
BEDROOMS: There were six (6) total bedrooms: four (4) designated resident rooms and two (2) designated staff rooms. Bedrooms #2 and #4 had a direct exit outside. Bedroom #3 had a door that led to a connecting hallway that had two (2) staff rooms and a restroom. Each bedroom had adequate lighting. The Administrator stated that Bedroom #3 will likely not be a resident room and plans to decrease capacity to four (4).
RESTROOMS: There were three (3) total bathrooms: one (1) private, one (1) shared, and one (1) staff. Bathrooms are in operating condition with grab bars. Hot water temperature was tested and measured at 138.7 degrees F, which is above the required range. Administrator stated that family is currently living in the property and they prefer hot water, but the water temperature will be lowered before admitting residents and when family vacate.
