Facility Evaluation Report
Licensing Program Analysts (LPA) Brian Balisi arrived at the facility unannounced to conduct a required annual visit at 11am. Upon arrival LPA met with Head Caregiver Dexter Olvario and explained the reason for the visit. Administrator Belinda Dolinsky could not be on site at the time of the visit, but Dexter Olavario was designated to sign in their place. The LPA toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.
DINING ROOM / KITCHEN: The LPA began the inspection in the kitchen/food service area at 9am Knives and sharp objects were located locked and inaccessible on the top drawer on the kitchen island. Kitchen appliances were observed to be in operable condition. The facility has a sufficient supply of perishable and non-perishable food. LPA observed disinfectants and cleaning supplies locked and inaccessible to residents in care. Medication cabinet and facility files located in cabinets to the right of the fridge were observed locked and inaccessible to residents in care. At approx 11:05am LPA observed (3) residents watching television in the living room. The door leading into the attached garage was observed inaccessible to residents during the visit.LPA observed garage to store cleaning supplies, additional Personal Protective Equipment (PPE) and incontinence supplies.
COMMON AREAS: LPA inspected the common areas throughout the facility. The common areas include the living room and family room. All the rooms have been appropriately furnished. There is a dedicated area for the posting of required documents located in the corner office area in the living room. The common areas were observed to be properly furnished and relatively clean at the of the visit. LPA observed appropriate signage regarding infection control posted throughout the facility. LPA observed sanitizer readily available in areas with high touch surfaces. Dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detectors were operational at the time of the visit. Fire extinguishers were observed fully charged and were last serviced July of 2023.
