Complaint Investigation Report
It was alleged that facility is not clean, sanitary, or in good repair. It was reported that dish sponges were filthy, leftover food was left in the kitchen sink, and the pots and pans were rusty. It was further reported that mattresses were stained and there was mold in the shower. During the plant tour, the LPA observed client bedrooms, including bedding; client restrooms, including the bathtub and shower; as well as the kitchen, kitchen sink, and the supply of pots and pans. The LPA found the facility to be relatively clean. Client bedrooms were organized, and the mattresses were clean and had clean linens as well. The bathrooms were clean and sanitary, and both the shower and bathtub appeared clean with no signs of mold or mildew. The kitchen sink was clean, and all pots and pans were in good condition. Additionally, the dish sponges showed no visible residue. Interviews conducted with staff revealed that the facility provides housekeeping services at least once every two weeks, during which a deep cleaning of the entire facility is performed. Staff and clients assist in maintaining cleanliness between these services. Staff further stated that nocturnal staff typically clean and mop during their shift while clients are asleep. A client interview corroborated the staff statements, confirming that the facility is consistently clean and that cleaning services are provided regularly. Based on information provided through interviews and observations, the Department has insufficient evidence to support the allegation of “facility is not clean, sanitary, or in good repair”. Therefore, this allegation is deemed Unsubstantiated at this time.
It was also alleged that facility did not address bug infestation. It was reported that there was a bug infestation in the rooms, beds, and kitchen. Record review and interviews conducted revealed that the facility has a monthly service with Tony Rubia’s Pest Control Company. The facility has both the interior and exterior perimeters treated to prevent bugs. Interviews with staff revealed that concerns regarding bed bugs had been raised. In response, the facility arranged for the pest control company to inspect and treat the premises. However, the pest control company informed staff that there was no evidence of a bug infestation, specifically bed bugs. The facility also had the client’s furniture cleaned to provide reassurance. Both staff and client interviews confirmed that there is no bug infestation in the facility and that the facility is maintained clean at all times. Based on the information provided, the Department has insufficient evidence to support the allegation of “facility did not address bug infestation”. Therefore, this allegation is deemed Unsubstantiated at this time.
