Complaint Investigation Report

It was alleged that the licensee did not display the facility license number in the advertisement, as the licensee did not include the license number on the facility website. Information gathered during the course of the investigation reflected that the facility was licensed effective 03/01/2024. A physical copy of the license was mailed by Community Care Licensing Division’s Centralized Application Bureau (CAB) on 03/14/2024 and was received by the licensee on 03/18/2024. Interviews conducted reflected that the license number was included under the “FAQ” page previously and was updated on 03/31/2024 to be on the main landing page of the website. A review of the facility website reflected both CCL license number and DHCS certification numbers on the bottom of the landing page. Based on the information gathered, the Department does not have sufficient evidence to determine that the licensee did not display the facility license number in advertisements. Therefore, the above allegation is deemed UNSUBSTANTIATED at this time.

It was also alleged that the facility staff do not have proper clearance and the facility is hiring people with criminal backgrounds. During the course of the investigation, LPAs reviewed eight (8) staff files and also reviewed the Guardian system for the facility roster. Based on the records reviewed and interviews conducted, all staff are required to obtain criminal record clearance prior to employment. All staff currently employed have the appropriate clearances and are associated to the facility. Based on the information gathered, the Department does not have sufficient evidence to determine that facility staff do not have proper clearance; therefore, the above allegation is deemed UNSUBSTANTIATED at this time.
