On 9/3/2025 at 1:45pm, Licensing Program Analyst (LPA), Perchui Milena Khurshudyan conducted an unannounced Case Management visit to amend the complaint report: LIC9099A and LIC9099-Cs recorded during a visit date conducted on 08/08/2025.
Upon arrival LPA met with the Caregiver Anahit Grigoryan and explained the reason for the visit.
