Facility Evaluation Report

At 1:10pm, Licensing Program Analyst (LPA) Perchui Milena Khurshudyan conducted a Case Management – Other /Closure visit and met with the Administrator designee Sulaimon Ajayi. The facility Administrator got contacted over the phone and informed LPA that due to the heavy rain and traffic he/she is running late. An entrance Interview was conducted.

LPA conducted a physical plant walk through and observed that the facility was vacant, and no staff or clients were present. The facility closure date is 11/1/2025 and it was confirmed during today’s visit. The Administrator trashed the original license, therefore, LPA was unable to collect the License during today's visit. Administrator will email the copy of the License to LPA. The Licensee/Administrator was reminded that he/she is not to admit/retain any clients at the home who require care and supervision.

LPA Khurshudyan will submit the file for closure upon returning to the Woodland Hills South Regional Office.
