Facility Evaluation Report

Licensing Program Analyst (LPA) Jose Tan conducted an unannounced case management visit to this facility to ensure that the facility has totally ceased operation and no client is no longer living at the facility. LPA met with staff Michael Custodio and explained the reason for the visit.

At 9:15 AM, LPA toured the facility inside and out with staff Michael Custodio and the following was observed:

There was no one left at the facility, staff or resident. LPA was informed by the operator that the physical license will be mailed to CCL as it was not present at this location at this time. The last resident moved out only yesterday to a licensed facility and therefore the effective date of closure is today.
