Facility Evaluation Report
At 10:00 AM Licensing Program Analyst (LPA), Lorena Casillas, conducted an unannounced annual inspection at the facility mentioned above. LPA was greeted by staff member, who granted access. Staff contacted the Administrator, Loida Quijano, who arrived shortly after. LPA explained the reason for the visit and an entrance interview was conducted.
A tour of the physical plant was conducted with the Administrator at 11:15 am. The facility has four (4) bedrooms, an additional room used as staff room and three (3) bathrooms. Three (3) rooms are used for clients with one (1) room being a shared room. Bedroom #2 is used as an office. Facility is fire cleared for four (4) ambulatory clients only. The following was observed:
Kitchen: LPA toured the kitchen at 11:20 am. LPA observed there to be sufficient stock of two-day perishable and seven-day non-perishables foods. The menu was posted for review. Frozen foods are properly wrapped and stored. Food storage and preparation areas are clean and inaccessible to pests. LPA observed the knives and chemicals locked in a cabinet and observed the medication cabinet locked and inaccessible to clients in care.
Common Areas : LPA toured all common areas of the facility. LPA observed common areas to be clean and furniture to be in good repair. The facility maintains a comfortable temperature at 72˚ F. LPA observed fire extinguisher to be full and last serviced on 04/24/2024, service company will re-service extinguisher on 4/30/25. At 11:30 am, staff tested the carbon monoxide and interconnected smoke detectors, all were functioning properly. First aid kit had Licensing requirements and is located in the medicine cabinet.
