Facility Evaluation Report
Licensing Program Analyst (LPA) Leslie Ngo-Castaneda met with facility staff Comfort Oviri for a case management. At 4:18 PM, licensee Joseph Jose arrive and was advised the reason of the visit.
The purpose of the case management visit is to make sure technical violation were corrected on reports issued on 5.22.2025
At 3:50 PM LPA toured the home and requested the following:
-CCR 80088(e)(3) Faucets used by clients for personal care such as shaving and grooming shall deliver hot water. (3) All toilets, hand washing and bathing facilities shall be maintained in safe and sanitary operating condition. Additional equipment, aids, and/or conveniences shall be provided in facilities accommodating physically handicapped clients who need such items.
POC: Based on LPA observation, the bathroom window were deteriorating and the bathtub has a spot of rust, which poses/posed a potential health, safety or personal rights risk to persons in care.
POC date 7.21.2025: facility repaired window and bathtub.
LPA also gather necessary documents for facility floor sketch and changes to the facility floor plan. Exit interview conducted, copy of this report signed and delivered.
