Facility Evaluation Report

On 02/20/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced case management visit to the above facility regarding deficiencies issued on 10/08/25 during the Annual Inspection. LPA met with Administrator Jun V. Flores as the purpose of the visit was explained.

During today’s visit LPA Villegas observed that the following citations were not cleared by POC due date which was 10/22/25. On 10/22/25 Administrator contacted LPA Villegas and requested an POC extension which was granted by LPM Hammond and POC due date was updated to 10/28/25, however citations were never cleared.

On 02/20/26 the following citations are being re-cited on 809D page(s): 85064(b), 80066(e), 80066(a)(10-11) , 80075(f) , 80076(a)(7)(15)(17), 80087(a)(1) , and 80069(b-c)(1) .
