Complaint Investigation Report

Executive Director indicated R1 is not responsible for the full $1,182.28, however there is an outstanding balance of $182.28 that R1 is responsible for due to past balance. Review of Billing Statements confirms this balance. R1 will not be charged Room & Board charges for the month of July 2022. Executive Director indicated R1 will be receiving this new updated bill with the correct charges.

Based on statements and interviews conducted with staff, residents, review of resident files and facility file records, there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
