Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an announced Case Management- Deficiencies visit on 07/16/2026. LPA was met by Administrator Brian Buenviaje and explained the purpose of the visit. During today’s visit LPA Ramirez requested and reviewed client#1 (C1) client file. During record review LPA Ramirez observed that C1 was admitted into the facility on 02/27/2009 and as of today, is 72 years old. Review of C1’s last medical assessment was dated 02/15/2015. Per California Code of Regulations , Title 22, Division 6 and Chapter 6, Acceptance and Retention Limitations- 85068.4(e ) : The licensee shall ensure that the medical assessment for each client 60 years of age or older is updated at least annually and in accordance with the regulations addressing medical assessments in Residential Care Facilities for the Elderly (RCFE) [California Code of Regulations, Title 22, Sections 87458(b) and (c)].

Based upon records reviewed, C1’s last medical assessment was not conducted annually as per the above regulation. LPA Ramirez was provided with a copy of C1’s recent follow-up and general wellness visit, however, this progress note does not identify C1’s results of any communicable disease and other criteria specified in medical assessments. LPA Ramirez issued one (1) deficiency based on record review.
