Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Visit-Deficiencies on 08/19/2025. LPA was greeted by House Manager Joel Alonzo and explained the purpose of the visit.
During staff interviews, LPA Ramirez discovered that the facility had a change in Administrator but did not notify this licensing agency, in writing, within 30 days of change of administrator, as required per 85061(b)(1)(2)(3). Interview with alternate Administrator Brian Buenviaje corroborated staff interviews. Staff interviews revealed the facility administrator Rosalinda Buenviaje has not been actively at the facility since 2022 but alternate Administrator Brian Buenviaje, would step in occasionally to assist. Facility staff could not provide LPA Ramirez with documentation to support the facility has as active administrator on the premises the number of hours necessary to manage and administer the facility. Staff interviews also revealed staff#1 (S1) has stepped in as “acting administrator” and is present at the facility most days. Per 85064(e), “ The administrator shall be on the premises the number of hours necessary to manage and administer the facility in compliance with applicable law and regulation .” Based on interviews conducted and records reviewed, LPA Ramirez will issue two (2) Type B deficiencies for violation of 85061(b)(1)(2)(3) and 85064(e).
Two (2) deficiencies were cited during this case management visit. Exit interview was conducted. A copy of this report, 809-D and appeals rights was provided via email.
