The inspector’s account

Licensing Program Analyst (LPA) Galarza conducted an unannounced Required- 1 year visit focusing on COVID-19 Infection Control Practices. LPA explained the purpose of the visit to Case Manager Blanca Brambila. Director of Regional Administration Rosalind Ford was explained the purpose of the visit telephonically. The Day Program is licensed to serve 75 ambulatory, of which 12 may be non-ambulatory developmentally disabled adults ages 18 and over. At this time the day program is only providing traditional community based programming services to 49 clients, and alternative programming (Zoom or partial days) to 44 clients.

Facility is a one story building. The program consists of a lobby area, eight (8) training/activity rooms [arts/crafts, game room, library, salon, IPAD room, media, fitness room, and quiet room], 2 restrooms [changing room in 1 restroom], 6 staff offices, conference room, kitchen, staff lounge, storages rooms, and 2 exits. There is an outdoor gated patio area with three (3) shaded patio tables with benches. Facility provides transportation services, and is equipped with 10 vans & 2 buses (1 is not operable at this time).

Electrical smoke detectors, and sprinklers were observed. Carbon monoxide detector was observed. A pull-switch / fire alarm sounding device was observed in the kitchen, and main entrance of the facility.

Bathrooms were observed to be operational, clean, and ADA approved to accommodate non-ambulatory persons in wheelchairs.

The last fire emergency drill was conducted on 11/21/2022. The facility's last fire inspection was conducted on 1/19/2022 by SSD Alarm System Fraker Fire Protection.
