Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Annual Required Visit on 09/29/2025. LPA Ramirez was met by Administrator Yanira Rivas and explained the purpose of the visit. House Supervisor Jaime Santiago-Tovar arrived shortly after to assist with tour. The facility is licensed to serve four (4) ambulatory and two (2) non-ambulatory Developmentally Disabled clients, ages 18-59. All clients residing at this home receive case management services provided by East Los Angeles Regional Center.
Infection Control- Staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.
Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected four (4) client rooms. All client bedrooms contained required furniture, linens and lighting. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees.
Operational Requirements- Fire clearance is approved for four (4) ambulatory and two (2) non-ambulatory Developmentally Disabled clients, ages 18-59. Licensee provides care and needs according to the clients’ needs. Outdoor space with plenty of shade was observed. 1:1 coaching from El Sol Medical Focus is provided for one (1) client in care . LPA Ramirez verified staff from El Sol Medical Focus are criminally cleared.
