The inspector’s account

It was revealed that all hazardous liquids stored in the backyard had been cleaned up. Per staff interviews, all three (3) staff interviewed could not corroborate the allegation which indicated all hazardous liquids in the back yard, side yard and driveway were disposed of after the Regional Center visit in March 2026. During the physical plant tour, no hazardous liquids, cleaning supplies, toxic items, disinfectant items were observed in the back yard, side yard, front yard or driveway. Inside the facility, cleaning supplies, toxic items and disinfectants were locked and inaccessible to clients. Per LPA’s observation during the visit, staff would lock cleaning supplies immediately after use which was in accordance with Title 22 practices. Per record review, in-service training related to Title 22, Section 80087, for ensuring disinfectants, cleaning supplies and toxic items to be inaccessible to the clients was provided to all staff on 04/14/26.

Based on the information obtained during the investigation, interviews with staff/clients, review of client files and LPA's observation, the investigation did not reveal any evidence to support the allegations mentioned above.

Although the allegations may have happened or are valid, there is no preponderance of evidence to prove the alleged violations did or did not occur, therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted with Staff#2. The finding was discussed and a copy this report was provided.
