Facility Evaluation Report

Licensing Program Analyst (LPA) Nicol Wesley conducted an unannounced Required 1 year inspection at the facility and met with Rosario Garcia. The purpose for todays visit was explained. The facility phone number is 562 698 1608. The administrator Rosario G Nicolas allowed, staff Rosario Garcia to sign the report on her behalf.

The facility consist of 4 bedrooms(1 for staff), 1 1/2 bath rooms, living room, laundry area, kitchen, dining room, back yard with shaded area, den with office area, and detached garage(food storage). The CARE tools was utilized to conduct the visit.

LPA Wesley conducted a complete tour of the facility and observe the supply of food. Resident medications, and medication logs were reviewed. The smoke detectors/carbon monoxide detector are operable. LPA observed two fire extinguishers one in kitchen area, and the other in the hallway. The water temperature was tested and measured 114.3 degrees F. The last fire drill was conducted on May 13, 2025. LPA Wesley reviewed 3 resident and 3 staff files. The administrators certificate for Rosario G Nicolas expired on 04/21/2025. She sent a check for renewal on 04/21/25. .
