Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Visit-Incident on 03/24/2026. LPA was greeted by Administrator Lladira Perez and explained the purpose of the visit.

Upon arrival to the facility front gate entrance, LPA Ramirez observed the delayed egress gate to be in disrepair and not operational. The facility is approved for delayed egress and currently has clients that are at risk of wandering away and running away from the facility. Administrator Perez revealed that the delayed egress gate has been in disrepair since 03/17/2026 and a work order was placed 03/18/2026 to fix the gate.

Based on observations, LPA Ramirez will issue one (1) Type B deficiency. Exit interview was conducted and a copy of this report, 809-D and appeals rights was provided.
