Facility Evaluation Report
Licensing Program Analyst (LPA) Tena Herrera conducted a Case Management visit. LPA met Angela Boyd - Health Services Director and discussed the purpose of this visit.
During the course of a complaint investigation for complaint # 28-AS-20241113164508, it was discovered that the administrator at the time of the complaint investigation, did not ensure that staff were properly trained on how to operate a hoyer lift , which resulted in injuries to a resident in care. During investigation of above referenced complaint LPA reviewed staff files and did not observe any training's on file on how to utilize a hoyer lift properly while assisting residents (citations were previously issued during complaint investigation dated 12/18/2024 for injuries resident sustained, however, lack of training was not addressed at that time). Today LPA is addressing the and issuing the deficiency.
