The inspector’s account

Licensing Program Analyst (LPA) Erik Zaragoza conducted a subsequent case management visit in regards to a death report received for Client #1 (C1), who passed away on 3/17/2026. LPA met with Yolanda Amaya, DSP for the facility, and explained the purpose of the visit. Administrator Crystal Alegre was notified of the visit by phone call.

According to the death report, it describes that C1 had difficulty breathing, and that their physician recommended that they be taken to the hospital following testing on their breathing showed that it was abnormal. C1 had passed away at the hospital, however the cause of death is unknown.

During the initial visit on 4/2/2026, LPA requested that administrator Crystal Alegre submit C1's Physician's Report, FACE Sheet, Individual Program Plan (IPP), and their Medication Administration Records (MARs) for the months of February and March to LPA by email. LPA requested that the administrator submit a copy C1's death certificate to LPA once it became available. LPA also toured the bedroom of C1 and found no concerns.

Administrator Alegre submitted the death certificate to LPA for C1, and it reveals that they passed away due to Chronic Obstructive Pulmonary Disease.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted. A copy of this report, along with the appeal rights, was provided.
