Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Ramirez conducted a subsequent unannounced Case Management Visit- Incident on 03/05/2026. LPA was greeted by House Supervisor Keily Sandoval and explained the purpose of the visit.

This is a follow up visit to render a finding from Case Management Visit- Incident reported dated 02/03/2026. LPA Ramirez conducted five (5) staff interviews. One (1) out of the five (5) staff interviewed revealed that C1’s personal rights were violated on 01/22/2026. Due to cognitive impairments, LPA was unable to interview six (6) out of the six (6) clients in care. Review of client#1 (C1) file did not reveal C1’s personal rights were violated by S1 on 01/22/2026. On 02/03/2026, LPA observed staff providing care and supervision to two (2) out of six (6) clients in care and observed staff being professional and respectful with clients. LPA Ramirez observed two (2) out of the six (6) clients in care to be well groomed and well nourished. During today’s visit, LPA Ramirez spoke with Administrator Johnsrud via telephone. Administrator Johnsrud revealed that after an internal investigation, the alleged allegation that S1 violated C1’s personal rights was unconclusive. Administrator Johnsrud revealed that although their internal investigation was inconclusive, the facility relocated S1 to another facility.

No deficiencies were made during this visit. Exit interview was conducted and a copy of this report was provided.
