Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Annual Required Visit on 07/11/2025. LPA Ramirez identified herself and explained the purpose of the visit. LPA Ramirez was met by Administrator- Mariano Prieto. The facility is licensed to serve six (6) ambulatory Developmentally Disabled clients who are ages 18-59. All clients residing at this home receive case management services provided by East Los Angeles Regional Center.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control- Staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.

Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected four (4) shared client rooms. All client bedrooms contained the required furniture, linens and lighting. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees.

Operational Requirements- Fire clearance is approved for six (6) ambulatory clients aged 18-59 years old. Licensee provides care and needs according to the clients’ needs. Outdoor space with plenty of shade was observed. LPA Ramirez observed one (1) out of six (6) clients in the home receiving 1:1 day program service in the facility. Staff providing 1:1 coaching are from El Sol Medical Focus. LPA Ramirez verified staff from El Sol Medical Focus are criminally cleared. SEE 809-C
