Complaint Investigation Report
The investigation revealed the following: regarding the allegation “Staff inappropriately touched resident while in care.” On 03/09/2026, C1 allegedly reported that an unknown facility staff member inappropriately touched C1’s private area. Interviews conducted by Community Care Licensing Investigations Branch with facility staff, C1’s day program staff and C1, did not corroborate this allegation. Staff interviews conducted by LPA Ramirez did not corroborate this allegation. LPA Ramirez attempted to interview C1- C5, all attempts were unsuccessful. LPA Ramirez attempted to interview C6, due to C6’s behaviors and cognitive abilities, C6 was unable to provide a meaningful interview. Review of C1’s medical records and incident report revealed on 3/07/2026, C1 was vomiting and was taken to a local hospital. C1 was given an abdominal examination and released that day with new orders. Review of C1’s recent Individual Program Plan (IPP) and quarterly progress report revealed that C1 has a history of fabricating stories. Review of C1’s medical report from 03/07/2026 did not corroborate this allegation. Review of Community Care Investigations Branch Assignment Report did not corroborate this allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED .
No deficiencies were cited during this complaint investigation. Exit interview was conducted. A copy of this report was provided.
