Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit due to the death of a client. LPA arrived unannounced and met with Administrator, Russell Johnsrud, and explained the reason for the visit.

On 7/27/25, LPA received a death report for Client #1 (C1), who passed away at the hospital on 7/25/25, approximately 8 PM. The administrator reported that C1 was in the hospital on 6/19/25 for a scheduled second surgery due to the colon cancer diagnosis. The surgery was successful, however, C1 had complications during recovery. The family informed the administrator on 7/26/25 that C1 had passed away at the hospital.

During the visit today, LPA toured the facility and did not observe any health and safety concerns. LPA collected documents on C1 and requested that the death certificate be sent to LPA upon availability.

An exit interview was held, and a copy of this report was given to the administrator.
