Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Annual Required Visit on 07/11/2025. LPA Ramirez was met by Alternate Administrator Elid Duran and explained the purpose of the visit. Quality Assurance Direct- Claudia Lujan arrived shortly after to assist with tour. The facility is licensed to serve twelve (12) ambulatory and six (6) non-ambulatory developmentally disabled clients who are ages 18-59. The facility has three (3) separate homes with 3 different addresses: 10746, 10748 and 10750 on one lot under the same facility number. All homes are located in a residential area and each facility consists of 4 Bedrooms and 2 Bathrooms in each building. Each home is managed by a one (1) House manager.
Infection Control- Staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.
Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected four (4) shared client rooms. All client bedrooms contained the required furniture, linens and lighting. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees.
Operational Requirements- Fire clearance is approved for six (6) non-ambulatory clients aged 18-59 years old. Staff provides care and needs according to the clients’ needs. Outdoor space with plenty of shade was observed.
Staffing- Administrator Certificate for Rosa expires 11/09/2026. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility. Two (2) staff are on duty from at least 10pm to 6am to provide nighttime supervision.
