Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Ramirez conducted a subsequent unannounced Case Management Visit- Incident on 03/05/2026. LPA was greeted by Michelle Gozopuno and explained the purpose of the visit.

This is a follow up visit to render a finding from Case Management Visit- Incident reported dated 02/03/2026. LPA Ramirez conducted four (4) staff interviews. Four (4) out of the four (4) staff interviewed denied that client#1 (C1) personal rights were violated by S2. Interview with C1 did not corroborate that their personal rights were violated by S2. Review of client#1 (C1) file did not reveal C1’s personal rights were violated by S2. Interview with S1 revealed that S2 has not worked at the facility since 04/27/2025.

No deficiencies were made during this visit. Exit interview was conducted and a copy of this report was provided.
