Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Visit- Incident on 02/03/2026. LPA was greeted by Alternate Administrator- Michelle Gozonpuno and explained the purpose of the visit.
On 11/04/2025, LPA Ramirez received a Vendor Special Incident Report that revealed on 11/02/2025 at approximately 11:20am, client#1 (C1) conservator (W1) contacted Administrator Michelle Gozon Puno and advised her that after reviewing C1’s bank statements, it was discovered that C1 was paying $24.99 for a monthly subscription to Netflix. W1 contacted C1’s bank to have the automatic payments cancelled. W1 attempted to have the Netflix account canceled but was notified by Netflix that the account was not under C1’s name and therefore could not be canceled. S2 was asked about the alleged Netflix transactions and admitted that S2 and C1 only used the Netflix account one time so C1 and S2 could watch movies while at the facility. S2 denied opening the Netflix account when asked if they initially opened the account. S2’s last scheduled shift at the facility was on 04-27-2025.
