Complaint Investigation Report
Allegation: Staff are not meeting the required personnel requirements. It is alleged that two facility staff/Crisis Prevention Institute (CPI) certified training instructors do not have access to order training materials, log classes, or utilize CPI portal in any capacity. And as a result the staff are not meeting required personnel training requirements. Staff interviews revealed that the facility had two (2) CPI certified staff instructors until July 14, 2025. A total of 8 staff were interviewed. All staff stated they have continued to receive required 20 minute CPI refresher training and all other CEU monthly trainings. The Regional Center issued a Corrective Action Plan (CAP) dated 10/8/2024 that states the licensee shall certify a back-up CPI training instructor. During today's visit, a call to CPI Inc was made. The representative said the CPI instructor credentials are in good standing, instructor has access to materials, but cannot purchase more products until the account balance is paid. Licensee was interviewed and stated that they are exploring contracting an outside back-up CPI instructor, and/or considering appointing a facility staff as the 2nd CPI certified instructor. LPA contacted the Regional Center. The Regional Center stated a reasonable time frame of identifying a second CPI instructor should have been within 30 days. The Regional Center requested that 2nd CPI instructor shall have completed CPI certification by 9/30/25. Per record review, all staff have valid certification and monthly 20-minute refresher training. Therefore, there is not supporting evidence to confirm the allegation.
Allegation: Facility is experiencing a financial issues. The complaint alleges licensee has not paid CPI Inc [Crisis Prevention Institute, Inc] account invoices and there is an outstanding balance, which directly impacts the facility plan of operation. A total of eight (8) staff were interviewed. All staff stated they have received wages on time and do not think licensee has financial issues because utilities are paid for and client needs are met. One (1) staff stated they had knowledge of the outstanding CPI bill balance, but they assumed corporate office was aware. LPA called Licensee, they stated that CPI Inc has not contacted the licensee in reference to the outstanding balance. Licensee stated they will contact CPI Inc and pay the entire outstanding balance by tomorrow. LPA spoke to representative at CPI Inc, whom confirmed that the invoices have been mailed to the facility address, and not the licensee corporate office. In addition, CPI Inc did not have the licensee email on record, they had a former staff email as their contact information. CPI Inc stated correspondence regarding the outstanding balance was being sent to the former staff and not Deesha Inc (licensee). LPA reviewed Secretary of State records and found the licensee and their parent company to be in good standing. There is insufficient evidence to support the allegation.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED .
