Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Annual Required Visit on 09/09/2025. LPA was met by Administrator Erlinda Salvador and explained the purpose of the visit. The facility is licensed serve six (6) developmentally disabled clients 18-59 years old, of which all must be ambulatory only. All clients in the facility receive services from East Los Angeles Regional Center (ELARC).

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control- Staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.

Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to residents, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected three (3) client rooms. Three (3) out of three (3) the client bedrooms contained required furniture, linens and lighting. One (1) out of the three (3) client bedrooms is shared. LPA Ramirez observed an inhalation-assistive device in Client#2 (C2) bedroom. Records reviewed for C2 revealed C2 was prescribed the use of an inhalation-assistive device on 6/26/25. LPA Ramirez did review C2's recent Individual Program Plan (IPP) which indicates C2 was prescribed an inhalation-assistive device and is recommended to use it at night for 6 hours. Licensee will obtain a copy of C2's restricted health care plan regarding the use of the inhalation-assistive device and agreed to fax plan to LPA for review. LPA Ramirez will issue a Technical Violation based on record review. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees. SEE 809-C
