Facility Evaluation Report
***This visit is a continuation from attempted annual visit conducted by LPA Cota on 8/8/25 at 1:27 p.m.***
Licensing Program Analyst, Mayra Cota, conducted an unannounced annual visit and met with Loida Samonde, Manager and the reason for the visit was explained. Administrator, Martha Losoya, was contacted telephonically and was also informed about the purpose of the visit. Licensee, Joann Mocling, arrived thereafter and continued to facilitate the visit.
The facility is licensed to serve (4) ambulatory adults age (18) through (59) in room (1), (2), (3) and (4). There are currently, no clients residing in the home nor receiving care from the facility.
The single-story home is located in a residential area of Whittier, and it consists of the following: receiving area, living room, dining room, kitchen, laundry area, (4) client bedrooms, (2) full bathrooms, office, front and back yard and attached garage with a secondary laundry area.
Facility's indoor and outdoor premises are clean.
Passageways, walkways, ramps and exits are clear and free of debris and obstructions.
Furniture inside the home and in the back yard patio is clean and in good repair.
Appliances in the kitchen and the laundry areas are clean, sanitary and operable.
Kitchen is stocked with dishes, and other cooking tools and utensils.
Kitchen counter tops have small appliances which are also operable.
