Facility Evaluation Report

Licensing Program Analyst (LPA) Galarza conducted an announced Case Management visit. The purpose of the visit was to follow-up on the facility closure request. On November 13, 2025, the department received written notification of facility closure. The purpose of the visit was explained to Licensee Lester Ellet and Kathy Lester.

The last resident was relocated on Saturday, November 22, 2025 and ceased operation the same day.

Resident relocation was approved by Eastern Los Angeles Regional Center. Resident (R1's) belongings have been removed.

Licensee was informed that the original license is not transferable. A copy of the original license was surrendered to LPA during the visit.

Confirmation of facility closure was provided to Licensee.

An exit interview was conducted with Licensee Lester Ellet. A copy of the report was issued.
