The inspector’s account

Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced case management visit to address an incident that was reported to the Monterey Park Adult & Senior Care Regional Office. LPA met with Diana Gonzalez, administrator for the facility, and explained the purpose of the visit.

The Monterey Park received a report explaining that on 8/13/2025, there were strange noises coming from the facility and that police presence was noted, however the Monterey Park Regional Office never received an Serious Incident Report (SIR) regarding this incident. LPA interviewed the administrator who explained that on 8/13/2025, Client #1 (C1) had called 911 on themselves during a mental health crisis, and also attempted to harm one of the female staff at the facility. Police were called to the facility along with paramedics as well. The administrator explained that a similar incident also occurred yesterday 8/25/2025. LPA reviewed the record of an that an SIR was completed by the administrator, however it was not fully submitted and received by the department. LPA advised to fax the SIR to the department along with all additional related documents.

LPA also conducted a tour of the physical plant for the facility, no health or safety concerns were observed.
