The inspector’s account

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced required Annual Inspection on 04/02/2026. LPA was met by Direct Support Professional I – Minerva Cobilla and explained the purpose of the visit. Alternate Administrator Michelle Gozon-Puno arrived shortly after. The facility is licensed serve six (6) developmentally disabled clients 18-59 years old, of which all must be ambulatory only. No firearms are stored in the facility. LPA requested and obtained a copy of the facility liability insurance and surety bond. Clients were not available for interviews during visit due to being in their day program.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control- Staff are using appropriate hand hygiene etiquette and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.

Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms. Staff successfully tested detectors and LPA observed detectors to be operational. LPA Ramirez inspected three (3) client bedrooms. All bedrooms contained required furniture, linens and lighting. Client bedroom#2 sliding door was observed to be in disrepair. Sliding door was not locking and staff had difficulty sliding the door. The facility was previously cited on 10/22/2024 for this disrepair. LPA issued a citation based on this observation. LPA did not observe night light in hallway that led to nonprivate bathroom. Client#2 (C2) is visually impaired and does not have a private bathroom. LPA issued a citation based on this observation. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees.
