Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced required Annual Inspection on 04/09/2026. LPA was met by Alternate Administrator Michelle Gozon-Puno and explained the purpose of the visit. The facility is licensed serve six (6) developmentally disabled clients 18-59 years old, of which all four (4) may be ambulatory and two (2) non-ambulatory. No firearms are stored in the facility. LPA obtained copy of facility Emergency Disaster Plan (LIC 610D) & liability/surety bond.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control- Staff are using appropriate hand hygiene etiquette and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.
Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms. LPA Ramirez inspected three (3) client bedrooms. All bedrooms contained required furniture, linens and lighting. All client bedrooms are shared. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees. During annual inspection, LPA did not observe night-lights in hallway that led to a non-private bathroom. Interview with Alternate Administrator Gozon-Puno revealed she had night-lights in her possession to install in the hallway. Administrator Gozon-Puno presented LPA with the night-lights and invoices. Administrator Gozon-Puno installed night-lights during LPA's visit.
