Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Annual Required Visit on 10/28/2025. LPA Ramirez was met by Assistant Home Supervisor- Kristina Schotel and explained the purpose of the visit. Administrator Claudia Lujan arrived shortly after to assist with tour. The facility is licensed to serve six (6) developmentally disabled clients ages 18-59; of which four (4) may be non-ambulatory and two (2) may be ambulatory. All clients residing at this home receive case management services provided by East Los Angeles Regional Center.
Infection Control- Staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.
Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected three (e) client rooms. All client bedrooms contained required furniture, linens and lighting. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees. LPA Ramirez observed video surveillance on the outside perimeters of the facility. LPA Ramirez observed ants in bathroom#2. During record review, LPA Ramirez observed a work order request submitted by facility staff on 10/24/25, which indicated ants were observed in the facility and pest control services were requested. During today’s annual inspection, maintenance services arrived at the facility to address ants. LPA Ramirez will issue one (1) Technical Advisory based on observation and record review.
Operational Requirements- Fire clearance is approved for six (6) developmentally disabled clients ages 18-59, of which four (4) may be non-ambulatory and two (2) ambulatory. The Licensee provides care and needs according to the client’s needs. Outdoor space with plenty of shade was observed.
Staffing- Administrator Certificate (7032687735) for Claudia Lujan expires 06/15/2027. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility. Two (2) staff are on duty from at least 10pm to 6am to provide nighttime supervision.
