Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual inspection today. LPA met with Amalia “Amy” Yanson, Caregiver, and the reason for the visit was explained. Marilou Yanso, Administrator, was informed regarding today’s visit via telephone call and Amalia Yanson continued to facilitate the visit.
The facility is licensed to serve (6) developmentally disabled adults ages 18 to 59; (2) ambulatory and (4) non-ambulatory. The facility is operating within the scope of its license. There are currently (5) clients living in the home. The facility provides services to clients through the Eastern Los Angeles Regional Center.
The single-story home is in a residential area of Whittier, and it consists of living room/dining area, (3) client bedrooms, (1) full and (1) half bathroom, (1) staff room, detached garage with a laundry area, and front and backyard.
The facility was observed clean inside and out. Walkways, passageways and exits are kept clear of debris and obstructions. Furniture is clean and in good repair. Living room and dining area have sufficient seating for staff and clients. The kitchen was observed clean and appliances were operating properly. The facility has sufficient 2-day perishable and 7-day perishable supply of food. Sharps/knives, cleaning supplies and other toxins are kept locked in the kitchen. Bedrooms have all the required furniture, bedding and adequate lighting. Bathrooms were also observed clean and sanitary. Water temperature was tested in both bathrooms and measured within 105 – 120 degrees F. which is within the compliance range. Bathrooms have safety grab bars and non-skid surfaces. Extra towels, personal hygiene supplies and bedding are kept in a closet. ***Continues on LIC 809-C****
