Facility Evaluation Report
On 5/16/2022, Licensing Program Analyst (LPA) Ngozi Nwaokoro conducted an unannounced 1-year annual inspection visit, with emphasis on infection control. Upon arrival to the facility, LPA met with staff Tracey Johnson. The purpose of today’s visit was explained. The facility is licensed to serve 4 clients with developmental disability. LPA inspected the entire facility, reviewed clients files, medical records and staff file.
LPA and staff Tracey toured the entire facility. The facility is a single-story home located in a residential area, which includes ) Facility currently has Four (4) bedrooms, two (2) full bathrooms w/walk in shower, a family room, living room, dining room, kitchen, a garage/ laundry space and an indoor/ outdoor activity area, and shaded area in the backyard.
All client rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for client personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. Hot water temperature properly measured at 111.5 degrees. Disinfectants, cleaning solutions, are inaccessible to clients. Outdoor and indoor passageways are kept free of obstruction. There are no pools, or bodies of water on the premises. Lamps or lights in all rooms were observed. Bathrooms are clean and operational. Toilets, hand washing, and showers/bathtubs are in safe, sanitary, operating conditions.
LPA observed the facility to be clean and appropriately furnished. Storage areas for personal hygiene, cleaning agents, toxins, and sharps were inaccessible to clients. The kitchen was inspected and there is a sufficient amount of perishable and non-perishable food available which is stored properly. The facility has two Fire extinguisher was inspected and charged. LPA observed a 30 day supply of client medications. Smoke and carbon monoxide detector is operational.
