Facility Evaluation Report

At 9:45 a.m., Licensing Program Analyst (LPA) Huma Rahimi conducted a Case Management – Other visit to confirm the closure of the facility. The LPA contacted Administrator Jeffrey Savella by telephone and explained the reason for the visit. The LPA was informed that he was unavailable and had designated Alvin Supe as Administrator Designee. The LPA then met with the Administrator Designee and explained the purpose of the visit.

During the visit, the LPA conducted a tour of the facility and confirmed that no residents were present and that operations had ceased. The closure of the facility was initiated by the licensee due to an intended change in business.

The LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office. The licensee was reminded that no residents requiring care and supervision may be admitted or retained at the facility.

An exit interview was conducted, and a copy of this report was signed and provided.
