Facility Evaluation Report

Licensing Program Analyst (LPA) Wendell Smith conducted a case management visit due to observations that were observed on a complaint visit on 3/2/22 . During the visit it was found that staff #1 (S1) that was working at the facility was not cleared or associated to work in the facility. S1 was left alone caring for the residents with no other staff working. LPA met with staff and explained the reason for this visit. LPA met with the licensee as LPA was leaving the visit and explained the reason for this visit.
