09/11/2024 and conducted by Evaluator Nicholas Reed

COMPLAINT CONTROL NUMBER: 31-AS-20240911123109

On 09/17/24 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced complaint visit. LPA met with Staff #1 (S1) and disclosed the reason for the visit. To investigate the allegation above, LPA interviewed Client #1 (C1) telephonically at 3:30 p.m. today, S1 at 3:45 p.m., Staff #2 (S2) at 3:55 p.m., Client #2 (C2) at 4:05 p.m., and the administrator at 4:15 p.m. and toured the facility inside and out at 3:35 p.m. Regarding the allegation "Staff yelled at a client" it was alleged that staff yelled at C1 for reporting an incident. Interview with C1 revealed they did not recall being yelled at by a staff member and everything has been good in the home. Interview with S1 revealed staff speak loudly to C1 due to their hearing impairment, but staff are never rude or yelling disrespectfully. S2 stated the administrator spoke with C1 recently about an incident. Interview with the administrator revealed they spoke with C1 ludly because they did not have their hearing aide in, but the administrator did not yell at them. C2 stated they have not heard any staff yelling. LPA did not observe any staff yelling today in the home. Based on interviews and observations, there is no evidence to support that staff yelled at a client. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. Exit interview conducted.
