Facility Evaluation Report

ON 8/1/2025 Licensing Program Analyst (LPA) Perchui Milena Khurshudyan conducted a Case management visit at the above facility to review the current property floor plan. Upon arrival LPA met with the Staff/Caregiver Maria Samson and explained the reason for the visit. Facility Administrator Laila Kulungu arrived shortly after. Entrance interview conducted.

At approximately 1:35pm LPA requested facility staff and clients’ rosters. This Case management visit was conducted to check if the facility had any recent construction or floor plan modifications done. During today’s visit, LPA conducted a physical plant tour inside and outside of the facility to review the current floor plan. LPA also requested a copy of the facility sketch including both inside and outside of the property.

During the physical plant tour LPA observed seven (7) bedrooms, of which six (6) are designated for clients' private use and one (1) for staff use only, and three (3) bathrooms of which two (2) are designated for clients' use and one (1) for staff use only.

LPA reviewed collected facility sketches and observed no changes were made to the floor plan or throughout the property. LPA also interviewed two (2) out of six (6) clients present during the visit, who confirmed since they moved to the facility in 2009 and 2019 no constructions were made and both clients also stated that all six (6) clients always had their own private bedrooms. No deficiencies were observed during this visit.
