The inspector’s account

Licensing Program Analyst (LPA) Tihesha Smith and Licensing Program Manager (LPM) Naira Margaryan made an unannounced case management visit to this facility. Licensing representatives encountered fire inspector Lindsay Pelegrini during visit. The licensee was not present; however, he communicated with LPM Margaryan through the Ring camera system. LPM Margaryan explained the purpose of the visit, and the licensee stated he would not be available to come to the facility due to prior commitments.

Recently, the licensee submitted a request for a new fire clearance to retain five (5) non-ambulatory residents and one (1) bedridden resident. Licensing records show that on 03/30/15, the facility was previously approved for five non-ambulatory and one bedridden resident. This case management visit was conducted to clarify the licensee’s new request.

Prior to the visit, Licensing representatives reviewed the facility’s previously approved sketch and fire clearance documents. These were compared with the newly submitted sketch and fire clearance request. Changes were identified between the original 2015 physical plant sketch and the recent submission. LPM Margaryan and LPA Smith discussed the updated fire clearance request with Inspector Pelegrini, who explained that the facility will not be approved for bedridden fire clearance at this time.

Inspector Pelegrini informed licensing representatives that she visited the facility on 06/15/26
