Facility Evaluation Report

licensing Program Analyst (LPA) Nune Margaryan conducted an unannounced annual visit. LPA met with staff Ngozi Ogu. LPA explained the reason for the visit. Administrator Angela Nwaka arrived shortly after and assisted with the visit. LPA used the infection control tool to evaluate the facility. The physical plant was inspected along with medications, food supply, and clients and staff records. The facility is licensed to serve developmentally disable clients between the ages 18 to 59 of which all be ambulatory. There are currently 4 clients residing at the home and receive services from San Gabriel / Pomona regional Center. The facility is a single-story home and located in a residential area. LPA toured the home and inspected 4 clients bedrooms, office / staff room, 2 bathrooms: one in the hallway and one in the staff room, living room dining area, kitchen, garage, backyard, and front yard. Laundry area observed in the garage. The front and backyard are well maintained and there are no pools or large bodies of water. Passageways and exits are free of obstruction. Client bedrooms and bathroom were checked. Each bedroom is equipped with the proper furnishings. Bedrooms also have sufficient closet space. The hot water temperature was tested at the bathrooms and in the bathrooms in the hallway measured at 129.7 degrees F. Extra linens, blankets, towels, and personal hygiene supplies were observed. Laundry detergent were observed locked in the hallway cabinet. The kitchen was inspected. There is sufficient perishable and non-perishable food. All the appliances are working properly but not clean. LPA observed that there is layer of grease on the kitchen cabinets and on the kitchen hood vent. Sharps are locked and are inaccessible to clients.
