Complaint Investigation Report
Regarding Allegation, Staff member inappropriately touched resident : It is alleged that one staff groped client while riding in the van to a medical clinic. C1 stated C1 was groped by S4 during ride to medical clinic. Administrator and 5 of 5 staff stated they did not witness allegation. C2 could not collaborate the allegation
Regarding Allegation, Staff failed to treat resident with dignity and respect : It is alleged that staff do not treat client with dignity and respect. C1 could not come up with an example of when this may have occurred. Administrator and 5 of 5 staff denied the allegations. C2 could not collaborate the allegation
NOTE: During visit LPA witness C1 being disrespectful and demanding to S1 S2 and S5 in loud voice. C1 threaten to destroy home if C1 did not get his way. C1 referred to C2 as retarded. C1 demanded to leave the home immediately in loud raised voice. C1 spilled a five gallon bottle of water on the floor and sink and yelled at the staff telling staff C1 will treat them like animals if they want to be treated like animals. C1 egged S5 to fight him and tossed water bottles at staff. C1 cussed at staff throughout the visit. .
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated UNSUBSTANTIATED
