The inspector’s account

The investigation revealed the following:

Allegation: “ Staff are hitting resident. ” I t is alleged that staff members are still hitting C1 on the head , but no injuries have been reported . (4) out of (4) staff interviewed denied the allegation and stated they treat clients with dignity and respect. S1-S2 stated they conducted an internal investigation a few weeks ago about a similar allegation and placed one staff member on administrative leave pending the conclusion of the investigation. S1-S2 indicated they have not received any complaints from clients regarding being hit and have not been contacted by the police. All (4) staff interviewed stated they have not seen any injuries on C1. S1 indicated that C1 has been treated by a doctor in 07/28/2025 and 08/25/2025 and was prescribed a cream due to scalp abrasion as C1 had scabs on the head from scratching. All staff interviewed confirmed they are trained on abuse/zero tolerance policy and are aware not to hit clients. S1-S2 also indicated that C1 has a history of fabricating stories. Clients interviewed denied the allegation and stated they have never been hit by any staff and have not seen any staff hit any clients. C1 stated that the incident about the staff hitting them on the head was a lie . C2-C3 indicated that staff are nice to them and they feel safe in the home. Documents reviewed revealed that C1 has a history of fabricating stories. LPA did not observe any injuries or bruises on C1 during the visit. LPA did not observe any injuries or any type of bruising on C1. Regional Center's Service Coordinator stated they unsubstantiated a prior allegation that was similar to this and they will look into this new allegation. Based on LPA’s observations, interviews, and record reviews, there was not enough supportive evidence to corroborate the allegation.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
