Licensing Program Analyst (LPA) Bennette Pena attempted an Annual/Required -1 year visit at the above-mentioned facility. Upon arrival at 1:08pm, no one was in the home to let the LPA in. The Administrator, Sang Heng was contacted by phone at 213-910-0442 and LPA was informed that they are out of town and will return the following week.
LPA will attempt to conduct a 2nd unannounced Required-1 year visit at a later date.
