The inspector’s account

Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced required annual inspection visit and met with Luisa Perez, Direct Support Professional. LPA Cota explained the reason for the visit and Perez facilitated today’s inspection. Roger Escorcia, Facility Representative, was informed of the visit telephonically and arrived thereafter.

The facility is licensed to serve (4) ambulatory developmentally disabled adults between ages 18-59 years old. The facility is operating within the scope of its license. There are currently (3) clients in care at the facility. Clients receive services through the San Gabriel/Pomona Regional Center. The facility is a single-story home located in a residential neighborhood of West Covina. The home consists of (3) client bedrooms, (2) bathrooms, living room, dining room, kitchen, laundry area, den, back yard, and detached garage.

The facility was observed clean, inside and out. All walkways, passages, and exits are clear and free of debris and obstructions. Living room and dining area have sufficient seating, and furniture is in good repair. Kitchen was observed to be clean. Kitchen appliances are also clean and operated at the time of visit. Cleaning supplies and toxins were observed locked in a closet and are inaccessible to client. Knives/sharps were also properly locked in a kitchen cabinet, along with clients’ medication. A sufficient supply of (2) day perishable and (7) day non-perishable food was observed. Food in the refrigerator and pantry is labeled and kept within expiration limits.
