Complaint Investigation Report
The investigation revealed of the following: Allegation#1 " Staff used profanity towards resident" LPA interviewed four clients and three out of four clients denied the allegation and reported staff never cursed at them or used any profanity words to them. They reported usually clients did that to staff. LPA interviewed staff and all denied the allegation and reported that they were all trained and were not able to talk back to the clients even clients screamed and yelled at the staff. The staff only try to redirect and calm them down.
Allegation#2 " Staff do not treat resident with respect" LPA interviewed four clients and three out of four clients denied the allegation and reported all the staff are very nice. No staff ever disrespect to them. Everyone is respectful. LPA interviewed staff and all denied the allegation and reported they would never being disrespect to the clients. Administrator reported that all the staff are professional and they would never insult the clients.
Allegation#3 " Staff hit resident with broom handle" LPA interviewed four clients and three out of four clients denied the allegation and reported no staff ever hit client with broom handle. The client was lied. LPA interviewed staff and all denied the allegation. Staff stated that no staff ever hit any client in the facility. They would never do that to any client. Staff stated client may hit back to the staff, but for sure not the staff.
Based on the interviews conducted with staff and clients and documents reviewed, the investigation revealed: Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are UNSUBSTANTIATED.
