The inspector’s account

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required-1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. Upon arrival, no one was home and the former administrator Herbert Bagoro was called who contacted the current Administrator to meet with LPA. At 2:00pm, current administrator, Bundi Nyaga arrived and LPA explained the purpose of today's visit. The facility is licensed to serve (4) developmentally disabled adults ages 18-59 years old, ambulatory only. The home is vendorized through San Gabriel Pomona Regional Center. Currently there are four (4) ambulatory clients, of which none have a restricted health care condition. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Staff are adhering to infection control requirements. The facility has submitted the Infection Control Plan. The facility is a single story home in a residential area, consists of kitchen, dining room, living room with covered fireplace, (3) client bedrooms, 1 1/2 bathrooms, laundry area, office area, attached garage, and backyard. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Smoke alarms and carbon monoxide were tested and operable. Laundry area is next to the kitchen area . Knives, cleaning solutions, and disinfectants are locked and inaccessible to clients. There is a fire extinguisher mounted on the dining area wall which was serviced on 01/17/2025. There are no firearms or weapons stored at the facility. Hot water temperature was measured at 117.5 degrees Fahrenheit in bathroom #1, and 116.2 degrees Fahrenheit in bathroom #2 which meets the Title 22 regulations. Exit doors are free of any obstruction and there are no pools or large bodies of water. Backyard was inspected and has a shaded area and sitting area. A fire clearance is in place. Surety bond is valid with a bond amount of $5000. Disaster drills are conducted quarterly, the last one being 01/12/2026.
